Protect the people in your care, and the people who provide it.

The most serious harm rarely comes out of nowhere. The signs are usually already in your data. PatternCare surfaces them early, so your participants are protected, and so is your service.

"Behind every incident report is a person who can't always speak up for themselves. I've seen how easily the warning signs get missed, not through neglect, but because no manager can stay on top of that much at once. I built PatternCare so those signs are caught in time, and the people in care are protected."
TAHLIA ELLAM · FOUNDER

Most major incidents start as patterns no one connected.

The biggest NDIS penalties came down to the same thing: risks that were already sitting in the provider's own records. Tap any case to see what happened, and how PatternCare changes it.

In each case, the warning signs were already recorded, just never connected. PatternCare exists to surface them while there's still time to act, for the provider and for the person at the centre of it.

Penalty figures and findings are drawn from public Federal Court of Australia decisions.

From incident report to actionable insight.

PatternCare is compatible with the incident and client management systems you already use. It reads what your team logs, finds the risk building across it, and keeps the proof. Click through each step.

Step 01 · Capture

Reads straight from the systems you already use.

PatternCare connects with your existing care management, incident reporting and support management systems, automatically capturing incident data as it's recorded.

Your team continues using the systems they know. PatternCare handles the analysis in the background.

Step 02 · Detect

Detects what your team misses.

PatternCare analyses every incident, identifies emerging and recurring patterns, and cross-references them against NDIS reportable incident definitions. By connecting incidents across participants, staff, locations and time periods, PatternCare identifies risks that would otherwise remain hidden across weeks or months of reports.

Most systems store incidents. PatternCare connects them, identifies risk patterns, and tells you what requires attention.

Step 03 · Act

Turn risk insights into action.

PatternCare flags incidents requiring immediate review and identifies emerging patterns that may develop into serious incidents if left unchecked. Every alert can be reviewed, actioned or evaluated, creating a clear record of oversight, risk management and compliance activity.

If something serious is logged as routine, it's caught here, before it's missed.

Built for providers who take risk seriously.

Whether you're a growing provider strengthening governance, a medium-sized provider preparing for audits, or a larger organisation managing risk across multiple teams, PatternCare helps you identify risk earlier, maintain oversight and stay audit-ready.

Flexible pricing based on provider size. PatternCare is designed to support both emerging and established providers, with pricing that scales alongside your organisation.

The honest answers.

I already record incidents in another system. Why do I need PatternCare?
Recording an incident doesn't mean a risk was identified, a pattern was detected or action was taken. PatternCare analyses incidents across participants, staff, locations and time periods to identify emerging risks before they become serious incidents. It also creates a complete record of reviews and actions taken, improving participant safety and providing stronger evidence during audits. No other platform combines pattern detection, risk classification and audit-ready reporting in one system.
What happens after an incident is logged?
Once an incident is logged through your existing systems, PatternCare automatically analyses the information for emerging and recurring risk patterns. Identified risks are surfaced within your dashboard, ready for review and action.
Is our information used to train AI models?
No. Your data is used only to analyse risk patterns within your own organisation. It is never used to train external AI models or shared with other providers. Your participants' information remains yours at all times.
How does PatternCare help with auditing?
PatternCare keeps a complete record as you go, so you're not rebuilding history at audit time. Export an audit-ready report in minutes showing every incident, its risk level, and how it was handled, with proof you stayed on top of risk throughout.
Can I bring my past data in?
Yes. When you join, we can import up to five years of your existing incident records, so PatternCare detects patterns from day one instead of starting from a blank page.
How do we get started?
From initial consultation to data import and onboarding, we can have you up and running within 3 to 4 business days.
Where is our data stored?
All data is stored within Australia and managed in accordance with Australian privacy and data protection requirements. Participant privacy and data security are central to the design of PatternCare.
